MANAGEMENT OF PATIENT HEALTH INFORMATION

All patient information is treated with respect and confidentiality.

Family Circle Medical Practice ensures all records are maintained and kept for the appropriate period.

Patients are able to access information in their files with the approval of the treating doctor.

ON-SITE PATHOLOGY

For your convenience, we have a Laverty Pathology collection centre at our practice.

Monday to Friday
8.00am – 12.30 pm; 1.00pm – 4.30pm

Parking and Accessibility

We have on-site parking, accessible from Denison Street. This entrance also has wheelchair accessibility.

Within the practice we have accessibility between levels by stairs and lift.

Payment for services provided is required on the day. We have EFTPOS and Credit Card facilities available for your convenience. The Medicare rebate will be credited into your account promptly.

A non-attendance or late cancellation fee of $50 for short consultations and $100 for long consultations applies if you fail to attend or cancel your appointment within 1 hour of the scheduled appointment time. Further appointments will not be offered until the fee is paid.

Due to COVID-19, home visits are not available.

Our practice offers telehealth and telephone consults for patients who are unable to attend the practice.

In the case of an emergency telephone 000.

For after hours appointments, call GP Access After Hours Service on 1300 130 147.

Alternatively telephone 13SICK on 137425 after 6pm and have an in-home visit from a GP.

Outside of Surgery hours, our telephone number will be directed to our voicemail service.

Emails are checked periodically throughout the day and will usually be actioned within one business day.

Please do not email us regarding an urgent matter.

If you need to speak with us urgently during our opening hours or do not receive a reply to your email, please contact us by phone.

We offer same day appointments.

We follow a strict appointment keeping system to reduce patient waiting times. 15 minutes are allocated per appointment. Longer appointments are available on request.

We believe time is precious and quality of life should be protected.

We strive to promote health and disease prevention to reduce the burden of disease.

We have a robust reminder and recall system in place for follow up of investigation results and preventative health care.

If you have questions regarding your results, make a follow up appointment with your doctor.

The doctor and staff will not be able to discuss health conditions or patient results over the phone in order to protect patient privacy.
