GP Chronic Condition Management Plan (GPCCMP)

What is a GPCCMP?

A GPCCMP is a comprehensive care plan developed by your GP to manage one or more chronic conditions. It outlines your health goals, treatment strategies, and any necessary referrals to allied health services. The plan is tailored to your individual needs and is reviewed regularly to ensure optimal care. Your GPCCMP will give you 5 allied health sessions per calendar year. These can be split among multiple providers, eg. 2 x audiology sessions, 2 x physio sessions, 1 x podiatry
session. You may reallocate your unused sessions to an alternative allied health provider during your GPCCMP review. Patients of Aboriginal or Torres Strait Islander descent can access 10 individual allied services per calendar year. GPCCMP holders will have access to subsidised allied health provider sessions, making them bulk billed or significantly discounted. Please check with your allied health provider to see what they offer as it varies between practices.

Who is Eligible? 

You may be eligible for a GPCCMP if you have at least one medical condition that has been present for six months or longer, or if you have a terminal condition. There is no specific list of eligible conditions; your GP will assess whether a GPCCMP is appropriate based on your health status. If you are unsure, speak to your regular GP at your next appointment, or have a chat to our friendly reception team. Patients can have a GPCCMP once per year, and a GPCCMP review every 3 months.
